*Copy of High School Diploma, Certificate of Completion or GED (This document must include a completion date.) *Copy of current resume *Copy of current EMT-B Certification (Renewed within the past 12 months prior to beginning of class.) *Copy of a current American Heart Association, Healthcare Provider, American Red Cross, Professional Rescuer CPR card or Military Training Network Healthcare Provider. *Proof of at least one year, full-time equivalent, or 1000 hours of EMT experience (Pay stubs, W-2 forms, letter from supervisor, etc.) *Two letters of recommendation (Professional – i.e. Supervisor, Nurse, Doctor, Fire Chief, etc.) *Completion of the Entrance Exam *Completion of a Paramedic Prep Class *Completion of an Anatomy and Physiology Class *Copy of Social Security card  Copy of valid Driver’s License UPON ACCEPTANCE INTO TRAINING CLASS, THE FOLLOWING REQUIREMENTS ARE DUE AT ENROLLMENT: *Provide a criminal background check from http://emstabackground.com *This is the only background check EMSTA College will accept for Paramedic students •Drug Screen through http://emstabackgroundcheck.com *This is the only drug screen EMSTA College will accept for Paramedic students *Successful completion of the application process Required Inoculations: *Annual tuberculosis screening: Mantoux tuberculosis test or if positive, Health Department’s guidelines for follow up. * Measles, Mumps, Rubella (MMR) *2 doses of vaccine documented or a titer evidencing immunity *Chicken Pox (Varicella) *2 doses of the vaccine documented or a titer evidencing immunity *Tetanus immunization within the past 10 years *Vaccine/immunity Hepatitis B is recommended *Seasonal Flu – can be declined if it is not available *Copy of current medical insurance coverage (policy or wallet card).
